The Client
Our client is one of Malaysia’s leading men’s health clinic groups, with physical locations spanning KL, Selangor, Penang, JB and Kuching. They provide specialist care across erectile dysfunction (ED) treatment, premature ejaculation (PE) therapy, and confidential STD testing and treatment — a category where discretion and trust decide where patients go.
They have been a Hypercharge Digital client for three years. When we started, their digital presence was a barebones placeholder site with zero organic traffic, and the business relied on over RM 50,000 per month in paid ads. Today they are Malaysia’s leading organic authority in men’s health — generating around 2,000 organic leads a month. This case study covers the next chapter: winning AI search.
The New Challenge: AI Search
Search behaviour changed. GEO doesn’t work on a weak site. The client’s AI dominance was built on the same fundamentals we had been executing since year one. Solid technical and on-page foundation. A ground-up overhaul that took the site from a 20/100 PageSpeed score and zero indexing to a fast, fully crawlable site with proper sitemaps, structured data and intent-matched service pages for every core treatment.

The GEO Programme
When AI assistants became a meaningful discovery channel, we didn’t start over — we extended the SEO engine with the generative engine optimisation playbook, built around three deliberate shifts:
1. AI-ready content and technical optimisation
We kept the same content cadence but restructured pieces for how LLMs read, applying entity-based SEO principles: direct answers high on the page, clear entity definitions, and question-led headings. On the technical side, we implemented llms.txt to guide AI crawlers, and submitted the site to Bing Webmaster Tools — a step many agencies skip, despite Bing’s index powering ChatGPT’s web search.
2. Prompt and query ideation
Classic keyword research maps what people type into Google. We built a parallel discipline: mapping the prompts patients actually ask AI assistants — 32 tracked commercial and informational prompts across ED, PE and STD testing — then engineering content to be the source AI models cite when answering them.
3. Increased off-page investment
LLMs recommend brands they see mentioned consistently across trusted third-party sources. We scaled up guest posting and brand-mention placements, prioritising the publications and directories AI models cite most often in this category.
The Client’s Contribution
The client’s team amplified the programme from their side — strengthening their social media presence and running PR activities that put the brand in mainstream media. These mentions compound: every credible reference to the brand becomes another signal AI models absorb.
The Results
We track the client’s presence across 480 live AI answers — 32 real patient prompts, tested continuously across ChatGPT, Gemini and Google AI Overviews.
How we measure AI visibility: 100 commercial and informational prompts a real patient would ask (e.g., “Recommend a reliable men’s health clinic in Malaysia”), run daily across ChatGPT, Gemini and Google AI Overview. For every answer we log which brands are mentioned, in what order, and which sources are cited.
The default recommendation, not just a mention
Visibility alone isn’t the goal — position in the answer is. When the client appears, it holds an average position of #1.7 in the recommendation list, and is the first clinic named in nearly half of those answers. Across all brands and platforms, the client holds the best overall average position (#2.2) and the #1 share of voice at 34% — meaning one in three clinic mentions in this category is our client.

Cited by the models themselves
Beyond visibility in answers, the site has earned 4.1k AI citations across 11 pages on ChatGPT, tracked via Bing Webmaster Tools — evidence that LLMs aren’t just naming the brand, they’re using its content as a source.
Strongest where money changes hands
Visibility is concentrated in commercial-intent prompts — the queries patients ask when they’re ready to book: 63% visibility for premature ejaculation therapy, 54% for erectile dysfunction treatment, and 49% for STD testing. The client holds a top-2 average position across all three treatment categories on non-branded queries, where the patient has no brand in mind and the AI’s recommendation decides the outcome.
Why It Worked
GEO is built on SEO. Three years of technical health, EEAT and topical authority gave AI models every reason to treat the client as the category authority. The GEO layer directed that authority at a new surface.
Off-page is the multiplier. LLMs recommend brands the wider web vouches for. Scaling brand mentions — agency-driven placements and client-driven PR together — moved the needle fastest.
